Resumen
Introducción: El tratamiento laparoscópico de la hernia paraestomal resulta muy controvertido debido a su alta tasa de recurrencias. No existe todavía consenso sobre cuál sería la técnica ideal. Se presenta una nueva opción técnica y se discuten los principios quirúrgicos que deben cumplirse en este tipo de hernia lateral.
Caso clínico: Describimos una nueva técnica que cumple los criterios de reconstrucción de toda la pared abdominal lateral con un nuevo tipo de malla: un polipropileno de baja densidad revestido de titanio. La técnica realiza un solapamiento completo de toda la hemipared abdominal posterior, una fijación combinada con grapas y cianoacrilato a sus bordes óseos (reborde costal y cresta ilíaca) y una tunelización de un segmento de asa mediante una malla con fenestración pero sin agujero central.
Conclusión: La reparación laparoscópica de la hernia paraestomal parece segura y eficaz. La técnica necesita ser estandarizada y centralizada. Se presenta una nueva opción que cumple los principios de las 2 reparaciones conocidas (la tipo «keyhole» y la de Sugarbaker), y añade el concepto de tratamiento integral de la hemipared lateral afectada.

Abstract
Introduction: Laparoscopic treatment of parastomal hernia is controversial because of its high rate of recurrence. There is still no consensus on what the ideal technique should be. We present a new technical option and discuss surgical principles to be observed for this type of lateral hernia.
Case report: We describe a new technique that satisfies the criteria for reconstruction of the entire lateral abdominal wall using a new type of mesh, a low-density polypropylenecoated titanium. The technique performs a complete overlap of the entire posterior half of the hemi-abdominal wall, consisting of a combined fixation with clamps and cyanoacrylate (to the costal margin and the iliac crest) and a tunneling loop segment by means of a mesh fenestration without a center hole.
